Reflux disease (GORD)

It is estimated that up to 20% of the population suffers from symptoms of reflux disease every week. Reflux is defined as when the stomach contents ascend into the oesophagus. It is associated with significant risks and complications such as ulceration, bleeding, stricture and cancer. There are very effective treatments available now to reduce the acid contents of the stomach. 40 - 90% of people will get good symptom control with these medications. However many people still get symptoms, get side effects or are reluctant to take a tablet long term. Most patients will need a gastroscopy at some point. There is effective simple surgery available that may be an option for these patients.

It is essential that the correct diagnosis is made in all patients as there are many other conditions that can cause similar symptoms. This is especially true if surgery is contemplated.

BRAVO capsule 

Because we are the centre of excellence for this condition, MacMurray gastroenterology is the only place in New Zealand that currently perform BRAVO capsule. A tiny capsule is clipped to the lower oesophagus during endoscopy. This stays un-noticed by most patients attached for 48 hours then falls off and passes away naturally.

The advantages are you can:
Eat normally
Shower normally
Maintain your normal life

A small pager sized device is worn and the data is sent wirelessly to it. After 48 hours you return the device and your specialist will read the information contained on it.
 

Potential risks and complications
Bravo capsule is not suitable for everyone. If you have a bleeding disorder, oesophageal varices, oesophagitis, stricture or a pacemaker then you should not undergo this test. You can not have an MRI within 30 days of the procedure.

Potential complications are:
Premature detachment of the capsule
Failure of detachment or discomfort requiring removal
Tears in the lining of the gullet
Bleeding
Perforation

pH and impedance

Catheter based pH recording with the addition of impedance is also offered through MacMurray gastroenterology. A tiny flexible tube is passed through the nose and sits in the oesophagus attached to a wire for 24 hours. Impedance allows us to measure all types of reflux that occurs and correlate this with your symptoms.
